Welcome to our Christian Hybrid Community! We're thrilled you're considering joining us for a unique, faith-centered educational experience. Together, we will create a nurturing and enriching environment where students can thrive academically, spiritually, and socially.

This application marks the beginning of our partnership in your child's educational journey, and we look forward to a successful year together.

We are excited to offer a unique, faith-based educational experience with a variety of program options for students in grades K-12. Our programs feature certified instructors/teachers, business trade professionals, and adaptive learner specialists to ensure a comprehensive and enriching learning environment.

Before completing this Membership Application, please read through Parent and Student Handbook to ensure that your goals and objectives align with our program. The handbook is a great resource and reference to the community, and we will build together as the foundational principles for the collective.

Registration is online. Please note after completion of this form you will proceed to select your course offerings.  Please be sure to take note of minimum requirements and supply fees. These will be displayed within the class registration process through our portal.  Please keep in mind that the membership enrollment fee will be charged per student selected upon completion of this form.  You may pay all at one time for check out.

Fees —

 

Information will be communicated directly from your instructor on any additional supplies needed. Lab costs will be additional for your course and will be provided in your course description. Please keep this in mind when registering.  Please be sure to read over course descriptions and costs in full and ask any questions prior to enrolling in the course.  

 

In addition, upon completion of registering, you will show a balance on the quantity of students slected for your membership.  Class selections will not be secured until ALL paperwork and fees have been received.

 

As part of our commitment to providing a high-quality, faith-based educational experience, our Christian Hybrid Homeschooling Community operates as a non-profit organization dedicated to supporting students and families. To sustain our programs and resources, we require a membership fee per student. This is per student enrolling into the community.

 

Your investment in this membership fee directly supports our mission to provide a nurturing and academically challenging environment grounded in Christian values. We are grateful for your partnership and look forward to a successful and enriching year together.

  • $300.00 Roaring Foundations Membership

 

Our unique a la carte course offerings allow students from K-12th grade to customize their education, choosing the classes that best meet their academic needs and interests. This approach ensures that every student receives a tailored education that aligns with their goals and values.

 

Our Christian Hybrid Community is dedicated to providing an educational environment where students can grow academically, spiritually, and personally. By offering courses a la carte, we ensure that every family can create an educational experience that is as unique as their child.

 

Building Fees: 
  • You build your program your way through the al la carte program. Every class will be added to your membership fee.
  • Example: $300.00+1 class $270=/if you take all 5 rotations it would be $1650
  • Enroll in 3 or more classes recieve a 5% discount off the class fee.  
  • *Lab costs will vary by subject, with biology and chemistry labs costing more than physics and earth science.

These fees can be paid directly when registering for the course. ESA debit card can be utilized for payment. More info will be provided upon completing registration.

 

Tuition Refunds:

1.  Membership Cancellation:


  • Notice Requirement: Families must provide written notice at least 30 days before the intended cancellation date.

  • Refund Eligibility:

    • If notice is given before the program start date, a full refund minus a $50 administrative fee will be issued.

    • If notice is given after the program start date, refunds are not available as resources and commitments have already been allocated.

2. Class Enrollment Cancellation:


  • Before Classes Begin:

    • Families who withdraw from individual classes prior to the start of the semester will receive a refund for class fees minus a $25 processing fee per class.

  • After Classes Begin:

    • No refunds will be provided.

    • Special exceptions (e.g., medical emergencies) will be considered on a case-by-case basis, subject to board approval.

3. Event or Workshop Cancellation:


  • Community Events and Workshops:

    • Cancellations made 7 or more days prior to the event will receive a full refund.

    • Cancellations made less than 7 days before the event will not be refunded.

4. Responsibility to Inform:


Families who cancel their membership or enrollment are responsible for returning any borrowed materials, ensuring outstanding payments are settled, and notifying instructors or team leaders of their withdrawal.

 

5. Program Discontinuation:


In the unlikely event that a class, program, or event is canceled by the community due to unforeseen circumstances, families will receive a prorated refund for any unfulfilled portions.

This policy ensures that we can maintain the quality of our programs while being good stewards of the resources entrusted to us. Thank you for your understanding and commitment to our shared mission.

    United Collective Statement of Faith


    At United Collective, we stand united in Christ and grounded in God's Word. Our community is built on the foundational truths of the Christian faith, and we joyfully affirm the following:

    At United Collective, we believe in the following core principles based on our Christian faith:

    • The Bible
      We believe the Bible is the inspired, infallible, and authoritative Word of God. It is the ultimate guide for faith, life, and truth (2 Timothy 3:16-17).

    • God
      We believe in one true God, eternally existing in three persons: Father, Son, and Holy Spirit. He is the Creator, Sustainer, and Sovereign over all things (Deuteronomy 6:4; Matthew 28:19).

    • Jesus Christ
      We believe that Jesus Christ is the Son of God, fully God and fully man. He was born of a virgin, lived a sinless life, died on the cross as the perfect sacrifice for our sins, rose from the dead, and ascended into heaven. He will return again in glory and power (John 1:1, 14; 1 Corinthians 15:3-4).

    • Salvation
      We believe that salvation is a gift of grace through faith in Jesus Christ alone. It is not earned by works but received by trusting in His atoning death and resurrection (Ephesians 2:8-9; Romans 10:9-10).

    • The Holy Spirit
      We believe the Holy Spirit dwells within all believers, empowering them to live godly lives, guiding them in truth, and equipping them for service and witness (John 14:26; Acts 1:8).

    • The Church
      We believe the Church is the body of Christ, composed of believers from every nation, tribe, and tongue. We are called to love one another, walk in unity, and share the gospel with the world (Ephesians 4:4-6; Matthew 28:19-20).

    • Marriage and Family
      We believe that God designed marriage as a sacred covenant between one man and one woman. The family is the foundational institution for nurturing faith and character in the next generation (Genesis 2:24; Ephesians 5:22-33).

    • Unity in Christ
      We believe in the unity of all believers through the Holy Spirit. While we may differ in secondary matters, we remain united in the essentials of the faith and commit to living in grace, truth, and love (John 17:21; Romans 14:19).


    Application of Faith in Education: At United Collective we integrate these foundational beliefs into our curriculum, activities, and interactions within the community.

Our primary concern at United Collective is the health and well-being of our students, staff, and families. This policy outlines guidelines to ensure a
safe and healthy environment for all members of our community.

Reporting Illness:

Parents are required to notify United Collective if their child is experiencing any of the following symptoms or conditions:

Fever:
  • A temperature of 100°F (37.8°C) or higher. Students must be fever-free without medication for at least 24 hours before returning to
  • classes.
  • Vomiting or Diarrhea: Students should remain at home until symptom-free for 24 hours.
  • Persistent Cough or Respiratory Symptoms: Especially if accompanied by fever or difficulty breathing.
  • Contagious Diseases: Such as chickenpox, measles, flu, or COVID-19.

Attendance Guidelines:
  • Students who are ill or experiencing symptoms should not attend on-campus classes or activities.
  • Parents are encouraged to keep their child at home if they are unsure whether they are well enough to participate in classes.

Health and Hygiene Practices:
  • Hand Hygiene: Encourage frequent handwashing with soap and water for at least 20 seconds, especially before eating and after using the restroom.
  • Respiratory Etiquette: Cover mouth and nose with a tissue or elbow.
  • when coughing or sneezing, and dispose of tissues properly.
  • Shared Items: Avoid sharing personal items like food, drinks, utensils, or supplies.

Medication Administration:
  • Mediation will need to be adminstered by the legal guardian and will not be done by anyone on campus.
  • In the event of a medical emergency 911 will be called to take medical measures.

Return to School Criteria:

Students may return to on-campus classes under the following conditions:
  • They are fever-free without medication for at least 24 hours.
  • Symptoms of vomiting, diarrhea, or respiratory illness have resolved.
  • They have completed any required quarantine period and are cleared by a healthcare provider if applicable.

Communication:


United Collective will notify parents of any suspected or confirmed cases of communicable diseases within the community while maintaining
confidentiality.


Parents are encouraged to inform United Collective of any health concerns or diagnoses that may affect their child's participation in on-
campus activities.

By adhering to these guidelines, we aim to create a safe and healthy environment where students can thrive academically and spiritually.

United Collective Parent Code of Conduct:


As a parent or guardian participating in United Collective, I understand that I am a vital part of this Christ-centered community. To help foster a safe, respectful, and God-honoring environment for all families, students, and staff, I agree to uphold the following standards:

1. Christlike Behavior


  • I will strive to model Christlike behavior in my words, actions, and attitudes.

  • I will treat all students, parents, staff, and leadership with kindness, respect, patience, and grace.

  • I will resolve conflicts biblically and respectfully, following the guidelines of Matthew 18.

2. Support for Leadership and Policies


  • I will respect the authority of United Collective leadership and instructors.

  • I will read and support the community guidelines, policies, and procedures, including dress codes, safety protocols, and communication policies.

  • I understand that all final decisions regarding classes, discipline, and community operations rest with the leadership team.

3. Positive Participation


  • I will arrive on time and ensure my student(s) are prepared, respectful, and ready to engage.

  • I will communicate promptly with leadership if my family cannot attend or participate as expected.

  • I will actively participate in volunteer roles or assigned duties with a servant’s heart.

4. Communication and Confidentiality


  • I will communicate honestly and constructively with staff and other parents.

  • I will avoid gossip, divisive speech, or public criticism of others in the group.

  • I will respect the privacy and confidentiality of other families and staff members.

5. Student Supervision and Conduct


  • I will ensure my child understands and follows the student code of conduct.

  • I will take responsibility for my child’s behavior and address any concerns in a timely manner.

  • I will refrain from disciplining other children and instead alert appropriate staff when needed.

In all things, I will seek to build up the body of Christ, serve others in love, and partner with United Collective to create a safe, joyful, and God-glorifying learning environment for all.

United Collective Student Code of Conduct:


At United Collective, we believe every student is created by God with purpose, value, and potential. To help build a safe, respectful, and joyful learning environment, all students are expected to uphold the following standards:

1. Be Christlike


  • Show kindness, patience, and respect to everyone—just like Jesus did.

  • Use words that build others up, not tear them down.

  • Forgive others and seek peace when conflicts arise (Ephesians 4:32).

2. Be Respectful


  • Listen when others are speaking.

  • Follow directions from teachers and leaders the first time.

  • Treat all people, property, and materials with care.

3. Be Responsible


  • Come prepared with the materials and attitude needed to learn.

  • Stay with your assigned group or class unless given permission to leave.

  • Complete assignments on time and give your best effort.

4. Be Safe


  • Keep hands, feet, and belongings to yourself.

  • Walk calmly and speak in a respectful tone inside buildings.

  • Follow all safety rules and procedures.

  • Encourage others and include everyone.

Emergency - Medical Treatment Information:

United Collective Permission to Release/Driving Students Authorization:

 

This section only needs to be completed if your child will be picked up by someone other than parents or legal guardian. If your child drives to school, you can skip this part as well.

 

Your child will not be released without proper identification of those authorized to pick-up. Please make sure those you are giving permission to pick up your child(ren) have a driver’s license or other proper photo identification to present when picking up your child. 
RELEASE, WAIVER, INDEMNITY AND HOLD HARMLESS AGREEMENT I/We hereby acknowledge that
I/We have read, understand and agree to comply with the provisions of this Release and Waiver. I/We,
United Collective Community (UC), its managing representatives, employees and other representatives
hereby waive any and all claims, potential claims, actions, remedies, whether in contract or tort, legal or
equitable against United Collective, its managing representatives, employees, heirs, assigns and/or other
representatives (collectively, the Released Parties), hereafter referred to as United Collective.
I/We, further agree to hold harmless and release United Collective, its managing representatives,
employees, heirs, assigns and/or other representatives from any such claims and actions that either I/we
and/or any guests, spouse, children that may arise from the usage of said facilities. I/We accept full
responsibility and assume all the risks and hazards associated with my/our usage of said facilities. In
consideration for the organization and allowing me/us to lease the facility, I/We hereby agree to
unconditionally release and agree to protect, indemnify and hold harmless, the organization, its managing
representatives, members, employees, heirs, assigns and/or other representatives (collectively, the
“Released Parties”) from and against any and/or all damages, injuries, illnesses, accidents, claims,
demands, causes of action and damages to or loss of property, including attorney’s fees, resulting from or
relating to my/our utilization of said facilities WHETHER OR NOT CAUSED IN WHOLE OR IN PART BY
THE NEGLIGENCE OF ANY ONE OR MORE OF THE RELEASED PARTIES. 
I/We hereby further conditionally covenant and agree that I/We, my/our spouse, legal guardians, heirs,
estate, legal representatives, successors and assigns will not make any claim or institute any suit or
action at law or in equity against any of the Released Parties that is based upon any injury, illness,
accident, incident, occurrence or damages to or loss of property arising out of, incidental to or in any way
resulting from my/our utilization of said facilities WHETHER OR NOT CAUSED IN WHOLE OR IN PART
BY THE NEGLIGENCE OF ANY ONE OR MORE OF THE RELEASED PARTIES. 
I/We acknowledge that I/we have read, understand and fully accept the terms and conditions of this
Release and Waiver and unconditionally and voluntarily accept the same. I/We have been advised to
seek counsel of my/our own attorney, before signing this document and have had the opportunity to do so
and have chosen of my own free will to sign this agreement. I/We agree that the laws of the State of
Arizona shall apply to the agreement and that any suit about this agreement or any underlying claim
covered by this agreement must be filed in the county of Maricopa State of Arizona. By typing my name below, I understand and agree that this form of electronic signature has the same legal force and effect as a manual signature.

United Collective Liability/Medical Consent:

Please read in its entirety and sign below:

 

I, the Parent(s)/Legal guardian(s) am allowing my child(ren) to take classes at United Collective, hereinafter referred to as United Collective in Buckeye, AZ. As such, I recognize the many benefits and personal responsibilities, which result from my child(ren) taking classes at United Collective. In consideration for these benefits and my own personal responsibilities to my child/children, I further agree to hold harmless and release United Collective and said leaders, teachers, and directors from and against any and/or all damages, injuries, illnesses, accidents, claims, demands, causes of action and damages to or loss of property, including attorney’s fees, resulting from or relating to my/our participation in a United Collective class, activity, or event (whether in the building, outside on campus grounds, or on a field trip) whether or not caused in whole or in part by the negligence of any one or more of the released parties.

 

I hereby waive any and all claims, suits, actions, remedies, whether in contract or tort, legal or equitable against United Collective, United Collective leaders, United Collective teachers, and United Collective directors that is based upon any injury, illness, accident, incident, occurrence or damages to or loss of property whether or not caused in whole or in part by the negligence of any one or more of the released parties.

 

I shall assume ultimate responsibility for the well-being, protection, and care to be provided to my/our child/children named above I further understand that I must remain accessible at all times by phone while my child/children are at United Collective (whether on campus or on field trips), as I am the sole responsible party for my child/children's medical care. In the event I cannot be reached, I hereby authorize any representative of said United Collective, to render first aid to my child(ren), transport him/her to a hospital or call an ambulance, and consent to medical treatment of my/our child/children in the event of an emergency (as determined by the representative).

 

 I acknowledge I have read, understand and fully accept the terms and conditions of this Liability Waiver and unconditionally and voluntarily accept the same. This consent is valid and irrevocable for as long as my child/children are enrolled in said United Collective. I further understand that any resulting charges or expenses will be fully my responsibility.

United Collective Fundraisers and Volunteering Efforts:
  • Students are required to participate in ONE fundraiser each year.
  • Students are highly encouraged to participate in quarterly service opportunities to assist with community involvement and partnerships.
  • Students are encouraged to track volunteer hours within the organization







By agreeing below, I certify that I have read, understand, and agree to the financial agreement set forth by United Collective. I acknowledge my responsibility to fulfill all financial obligations, including tutoring fees, class deposits, supply/lab fees, and any other costs associated with enrollment. I understand that failure to meet these obligations may result in penalties, holds on my account, or.withdrawal from United Collectives Program.   By typing my name below, I understand and agree that this form of electronic signature has the same legal force and effect as a manual signature.












































United Collective Classes Behavior Policy


The following rules help ensure a positive, safe, and honest environment for learning. Please review these items with your student(s). Any falsification of signatures (parent signing for the child) will result in immediate suspension of the student until the parent and child has met with a director and is approved to attend classes. 
  • Students will behave in a respectful and polite manner at all times. Any ungodly behavior (roughhousing, fighting, foul talk, harassment, etc.) towards others will not be tolerated. There is a no tolerance policy for credible threats of bodily harm.
  • Students must be in their class or in approved areas, such as study hall or lunchroom, at all times. Roaming the halls is prohibited. Absolutely no entrance into prohibited areas. When in the hallways, students must remain quiet and respectful of other classes.  
  • Respect for Property - Students will respect property of others, United Collective property, and Facility property. If a student destroys property of facility or United Collective, the family will be responsible for replacing it.  
  • Students are responsible for bringing all schoolbooks, necessary supplies, and completed homework to class each week. Students are responsible for their personal belongings at all times while on campus.  Students are responsible for cleaning up after themselves in classrooms, lunch, and study hall.
  • No weapons of any kind, including pocketknives, will be allowed on campus or in the parking lot. 
  • No smoking of any kind, including vaping, is allowed on campus or in the parking lot.
  • Clothing worn on campus shall be conservative and not distracting to others.
  • Modesty: Clothing should be modest, avoiding excessive exposure. No low-cut tops, midriff-baring shirts, or short skirts/shorts. Skirts and shorts should be at least knee-length.
  • Appropriateness: Clothing should be appropriate for a learning environment. Avoid clothing with offensive, inappropriate, or distracting messages or images.
  • Public displays of affection are limited to what is an acceptable, quick, friendly side-hug. We reserve the right to counsel students who blatantly go beyond what is deemed appropriate. We ask parents to discuss with their students respect of other students’ right not to be touched or hugged, and additionally, the inappropriateness of flirting.
  • Cell phones may not be used in classes and will be placed in the cell phone holder at the beginning of class. Cell phone calculators may NOT be used for Math / Science. If students use phones in class, they will receive one warning, then further use will result in confiscation of the phone. The phone can then be picked up at the end of the day from one of the directors.
  • Wi-Fi is not available for students. Only instructors may use Wi-Fi.
  • Cheating - In partnering with parents in the education process, we realize that families have differing values and priorities. In our classrooms, we observe a zero-tolerance cheating policy, as it dishonors God, the teacher, and the other students. If parents allow a student unsupervised access to answer keys for learning purposes, we welcome the student to audit the class, and the teacher will not issue grades or correct assignments. In the event that cheating is discovered, the student will be asked to sign a statement acknowledging that if they cheat again, or aid others in cheating, they will be suspended from class, or expelled, without a refund for missed classes. Teacher will determine time and length.
  • Study hall is a courtesy service for families. Students are to remain in study hall, without disruption, throughout the entire period. If they continually violate the rules, they will be asked not to return. The direction of the study hall monitor should be followed without question. Students are responsible for cleaning up after themselves.
  • Device sharing is prohibited.
  • Recording devices are prohibited on campus.
  • We have a zero tolerance for any type of bullying.

 

If an incident occurs, parents may be contacted for a meeting regarding the incident, and the Board along with the parent(s) will determine consequences. Repeat offenses will result in suspension or expulsion, to be determined by the Review Board. United Collective strives to maintain a safe and secure environment for all students and teachers conducive to learning. Any family whose students do not agree or continually disregard these policies should find another program to supplement their home school education.
